For prizes requiring a prescription, the sweepstakes entrants must be willing to provide a valid prescription within two weeks of winner contact from cpap.com or they forfeit the sweepstakes prize. For CPAP machine sweepstakes, a valid CPAP Machine prescription must be on file at the time of winner selection or winner must provide a valid prescription within 2 weeks of winner selection. For CPAP mask sweepstakes, a valid CPAP mask prescription must be on file at the time of winner selection or winner must provide a valid prescription within 2 weeks of winner selection. cpap.com will not ship the prize to the winner until a valid prescription has been provided to CPAP.com. Sweepstakes entrants must have or open a cpap.com account in order to submit their current CPAP prescriptions. All taxes on any sweepstakes item won are the sole responsibility of each winner, including, without limitation, any federal, state, or local taxes, which may be deemed applicable in such winner’s jurisdiction of residence. Winner(s) shall be solely responsible for the reporting and payment of all taxes incurred by acceptance and use of the prize(s) (or any portion thereof), if applicable.
