Forma Full Face CPAP Mask Assembly Kit
Mask Assembly Kits are a great way to refresh your mask without having to purchase the full mask. This is the Forma Full Face CPAP Mask Assembly Kit.
The Forma Full Face CPAP Mask Assembly Kit fits the "under the chin" and uses both a foam cushion insert and silicone seal. The t-shaped forehead support adds to the stability of the mask. This mask is available in three sizes, select the size needed when ordering.
The headgear is not included with the product. It can easily be added on the "Customize & Buy" screen by selecting Headgear for Forma Full Face Mask.

Where do I find the size?
The mask frame does not have an identifiable size because it is designed to fit each of the Forma Full Face cushions.
To find the size of the silicone seal, turn the mask to view the flat bottom of the full face mask. The user will find the size centered directly in the middle of the seal. Embossed letters spell "S M A L L," "M E D I U M," "L A R G E," or "X - L A R G E."
To find the size of the cushion, remove the cushion from the silicone seal. Hold the cushion with the soft edges facing away from the user. At the tip of the cushion, where the bridge of the nose meets the material, locate a small, embossed letter. This letter represents the size of the cushion. The mask frame does not need a corresponding letter as it offers a universal fit, but the silicone seal and cushion will need to be the same size to work as intended.
In the last 15 years the Forma mask is the only one I've not had an issue with regarding leakage due to my beard. However, the joint where the air valve attaches to the hose breaks easily. I have had to replace three masks in the last 9 months. I do turn from side to side at night but I have been using this mask for years and only recently had a problem with the mask actually breaking. more...
Struggling with this mask. Some of it is due to the fact I don't think I get it sealed to my face right every time and the other is that I seem to be allergic to the pads on the head gear. The bottom of the face mask seems to come out of the mask base and then the air rushes out the bottom. I have a small head and face so that doesn't help. One the nights it stays together it works good. more...
This Mask fits much better than the 432 that I had previously. This is a Great Item.
