Product Features

The Forma Full Face CPAP Mask Assembly Kit fits the "under the chin" and uses both a foam cushion insert and silicone seal. The t-shaped forehead support adds to the stability of the mask. This mask is available in three sizes, select the size needed when ordering.

The headgear is not included with the product. It can easily be added on the "Customize & Buy" screen by selecting Headgear for Forma Full Face Mask.

Important Tips

Where do I find the size?

The mask frame does not have an identifiable size because it is designed to fit each of the Forma Full Face cushions.

To find the size of the silicone seal, turn the mask to view the flat bottom of the full face mask. The user will find the size centered directly in the middle of the seal. Embossed letters spell "S M A L L," "M E D I U M," "L A R G E," or "X - L A R G E."

To find the size of the cushion, remove the cushion from the silicone seal. Hold the cushion with the soft edges facing away from the user. At the tip of the cushion, where the bridge of the nose meets the material, locate a small, embossed letter. This letter represents the size of the cushion. The mask frame does not need a corresponding letter as it offers a universal fit, but the silicone seal and cushion will need to be the same size to work as intended.
