  3. Click on the link that says “Fill Out Claim Form”. Your form will start downloading.
  4. Follow the instructions on the form, attaching your receipt and prescription.
  5. Submit claim form to your insurance provider.
  6. Your insurance company will evaluate your coverage and your claim and ultimately decide on what reimbursement you’re entitled to.

    I am a driver for my job and I have a doctors excuse I took a nice sleep study and they say that I need a CPAP machine searching around to find one I do have insurance
